• Choose Next and you’ll be redirected to the login page
• Sign in using the username and password you created

Once you’ve created an account, you’ll access the site in the future by going to CarePlusMobileHealth.com and signing in with your email username and password.

CarePlus Mobile Health is provided by Mobile Health Consumer (MHC). MHC collects information in order to provide suggestions on benefits available to you through their secure platform. Your name and your eligibility for services is provided by your Employer, or your Spouse/Domestic Partner’s employer. MHC may use health claims or biometric data from your health plan, insurance provider, or a third-party vendor to provide available services.

Mobile Health Consumer uses the information you provide or that it collects in order to establish and enhance its relationship with you. Mobile Health Consumer generally uses such information to operate, maintain, enhance, and provide all of the features and services found in the Services section.
